Medical Consent and Agreement (Required)

I, the undersigned, as the parent or legal guardian of the minor child named above, or as a participant (if over 18), hereby agree to the following: I give my consent for my child/myself to attend and participate in activities sponsored by Harvest Church of Alabama from June 23 to June 26 at the Town of McIntosh Park. I authorize an adult designated by Harvest Church of Alabama, who is responsible for the care of my child/myself, to consent to emergency medical treatment deemed necessary by licensed medical personnel. Efforts will be made to contact me immediately before such treatment is administered unless immediate attention is required. In the event of a life-threatening emergency, I authorize the administration of CPR and/or first aid by certified individuals until professional emergency personnel arrive. I understand that Harvest Church of Alabama and its affiliates are not responsible for injuries or incidents beyond providing normal supervision and care. These materials may include, but are not limited to, photographs, videos, printed publications, websites, and social media platforms. I waive any claims or rights to compensation arising from the use of these materials and release Harvest Church of Alabama, its affiliates, members, staff, and volunteers from any liability related to their use.
